I'm looking for my policy # ?

I have a blue cross of california insurance card that has my id card # and its 9 digits/letters long, i have a group # and its 6 digits/letters long, and then i have a coverage code that is 5 digits/letters long. I need to find a policy # for my physical form! please help, thanks!
