I got a questionnaire from my insurance. do i need to return it?

In november, my collar bone was broken, but it wasnt my fault. the assailant agreed to pay for my medical bills, but my insurance company now sent me a questionnaire asking me if the injury was my fault, i do not want to get back into it with the other person because i feel like the situation ended as well as it could. i was just wondering if i have to return the questionnaire to my insurance company.
