How to prepare for a Lymes disease specialist appointment?

I am a 25yr old female scheduled to see a lymes disease specialist next week for the first time ever. I tested positive for this 4 years ago, and I (mentally and physically) havent been the same since. I lose my health insurance next month, so in what ways can I prepare in order to get the most out of this important visit. Anyone that has been to one please tell me what to expect.
